Outreach Program
Program Description
Outreach focuses on creating awareness of the Aboriginal Health Centre and its services. It involves promoting health prevention and education through the coordination of the program initiatives with mainstream organizations and other Aboriginal agencies to improve relationships and continuity of services. Outreach includes developing, implementing, coordinating and evaluating programs that directly affect the Aboriginal community and assists in reducing access barriers to clients such as transportation.
Program Goal
To offer the services of the Aboriginal Health Centre to Aboriginal people residing in the Hamilton-Brant County Region, including more remote areas and hard to reach populations (i.e. homeless)
Program Activites
- Provide transportation assistance to and from the Health Centre when required
- Reimburse bus tickets for those with a low/fixed income for Health Centre appointments or programs
- Provide outreach services to hard to reach Aboriginal people in rural areas who have poor mobility, lack transportation and/or are not aware of the Health Centre's programs and services
- Network with existing providers in rural areas
- Provide culturally sensitive training to mainstream health service providers which serve significant Aboriginal populations (i.e. hospitals, clinics, shelters)
- In coordination with existing mainstream health services, provide culturally appropriate treatment and prevention services to hard to reach Aboriginal populations such as street people, substance abusers and transient populations
- Provide sterile needle exchange services
- Produce and distribute the Health Centre's newsletter
- Set-up displays and booths at special events, presentations, etc.
